Who We Are
Family-to-Family for Health Choices is Georgia’s Family-to-Family
Health Information and Education Center. The program is federally
funded, and was established to provide health information, training
and support to families of Children and Youth with Special
Healthcare Needs, and the professionals that serve them. F2F4HC is
a program of Parent to Parent of Georgia.
Our Mission: To foster a collaborative community
of families and professionals working together for positive health
outcomes for children and youth with special health care needs.
Children with Special Health Care needs defined:
Those children who have or are at risk for chronic physical,
developmental, behavioral or emotional conditions and who also
require health and related services of a type or amount beyond that
required by children generally. – (MCHB) Examples of such conditions
include: Asthma, Autism Spectrum Disorder, Cerebral Palsy,
Blindness, Loss of Hearing, Diabetes, Bipolar Disorder, Epilepsy,
etc.
Supports and services are organized around four levels: Thinking, Mentoring, Coaching and Leading. (Click on each level to learn more)
