Community Care Services Program (CCSP)

Community Care Services Program is the name of one waiver meant for people who have a disability.  It provides household and personal assistance and home health services.  This program is for persons with disabilities or functionally impaired elderly persons.  This waiver program does not pay for medical equipment, medical supplies or home modifications.  Recipients must be Medicaid eligible.  The average expenditure is $800 per month.  Applicant assessment is based on functional impairment and level of unmet need. The program serves adults and children (currently serving about 25 children).

Services include:

  • Respite Care

  • Alternative Living Services (Personal Care Home Services)

  • Adult Day Rehabilitation

  • Personal Support Services

  • Home Delivered Meals

  • Emergency Response
